Health Records

Upon entering Little Hands Big Hearts Preschool, a health record must be submitted containing the following information:

(   )Documentation of a physical, performed within a calendar year and signed by a physician

(   ) Documentation of any known allergies or chronic health conditions

(   ) Lead Test screened once before the age of 12 months and again at ages 2 and 3

(   ) Current immunization record documenting inoculations of the following:

 

MMR – Measles, Mumps, Rubella                 IPV – Polio

Hib – Haemophilus Type B                            DTP – Diphtheria

Varicella or disease of Chickenpox                Hep B – Hepatitis

 

*No child shall be required to have any such immunization if his/her parent objects thereto, in writing, on the grounds that it conflicts with their sincere religious beliefs, or if the child’s physician submits documentation that such a procedure is contraindicated.  We request that parents call LHBH when a child will be absent.  If a child comes to school and is not feeling well, he/she is more vulnerable to infection.  It is in the best interest of the child, other children, and staff, that the child remain home until well.  Please review the ILLNESS section on this website.
